Assist-Life Stage Transition
Support that helps participants plan and manage major life changes such as moving home, starting work, or transitioning to independent living. It focuses on planning, mentoring, and coordinating supports to build confidence and independence.
Alert Care Services Australia (ACSA) supports people with disabilities during major life transitions through its Assist-Life Stage Transition program.

The program provides individualised transition planning to help participants manage significant life changes such as moving between life stages, changing living arrangements, or developing greater independence. ACSA works with each participant to understand their goals, needs, and current supports, and then develops a structured plan to address potential challenges.

ACSA also assists participants to access relevant services and community resources, including healthcare providers, accommodation options, employment supports, and community programs. By connecting participants with appropriate service providers and advocacy organisations, ACSA helps ensure continuity of care and support.

The program promotes participant choice, control, and independence by supporting individuals to build skills, make informed decisions, and actively participate in planning their future.

Overall, the Assist-Life Stage Transition program aims to improve participant wellbeing, strengthen independence, and support successful transitions across different stages of life.
